Generated 6,900 unique tasks from the gap audit. The current plan has ten completed tasks, one active

||
pointer (`M16-GAP-00011`) and 6,889 pending tasks. Per-wave counts are M16 2,667, M17 29, M18 451,

||
M19 20, M20 89, M21 3,625, M22 19.

offset 读取单条记录并由任务卡提供真实命令。索引校验：`node tools/web/check-task-index.mjs`。
