+Generated 6,900 unique tasks from the gap audit. The current plan has ten completed tasks, one active
+pointer (`M16-GAP-00011`) and 6,889 pending tasks. Per-wave counts are M16 2,667, M17 29, M18 451,
+M19 20, M20 89, M21 3,625, M22 19.

+为避免执行者加载 6.8 MiB 的 pretty JSON，已生成 `tests/golden/M15-03A/task-index.json` 和
+`task-catalog.jsonl`。索引绑定上述 plan SHA-256，catalog 每行只保留任务身份字段；上下文工具按
+offset 读取单条记录并由任务卡提供真实命令。索引校验：`node tools/web/check-task-index.mjs`。
